Guide to Employee Surveys
(courtesy of Commute Trip Reduction program in the State of Washington) "This document provides practical advice to affected jurisdictions and employers on how to conduct surveys as efficiently as possible. Each section of the Guide to Employee Surveys takes you through the survey process, explaining how the survey is used and identifying important procedural steps. The appendices contain supplemental information, more detailed explanations, and examples of calculations and forms used in the survey process." (56 pages)

Surveying For Public Transit: A Design Manual for Customer On-Board Surveys
(courtesy of National Center for Transit Research) A best practices manual that describes the necessary steps in conducting a successful on-board survey of public transit customers. It was specifically developed for the public transit professional that has at least a rudimentary understanding of the purposes and procedures in survey research and is searching for specific guidance on how to “best” conduct an on-board survey of its customers. This how-to manual provides public transit professionals with a much better understanding of the total customer surveying process and its importance in planning and ultimately the highest quality service to the riding public. It describes the various components or steps of the on-board transit customer surveying process from specifying and clearly defined objectives, various methods of data collection, questionnaire construction, sample size, appropriate level(s) of analysis, accurate and truthful reporting of results, data entry, report writing, and data archiving.

Staff Travel Survey

Market Research Questionnaire

Employee Travel Survey

Ridematching Satisfaction
(courtesy of 511 Regional Rideshare Program) A customer satisfaction survey aimed at individuals who have used a ridematching service.

2003_Commute Patterns Questionnaire
A Commute Profile survey; it’s a region-wide telephone survey of commute patterns and commuter behavior.

Bike To Work Day Questionnaire
A follow-up survey of participants in its Bike to Work Day promotion. Designed to assess the effectiveness of the promotion.

Employer Transportation Survey
A standard employer–based transportation survey designed to help employers better understand the commuting patterns of their employees.

Telephone Questionnaire
A residential based telephone survey designed to understand travel patters and awareness level of residents in large development.

Paper Questionnaire
A questionnaire designed to track compatible data for employees in the same development as the previous telephone questionnaire.

Short employee commute mode survey

King County (WA) Commute Trip Reduction Employee Questionnaire
(courtesy of Washington State Department of Transportation) This survey is used to collect travel behavior data to comply with the state's Commute Trip Reduction law.  The King County version of the current CTR employee survey differs from the forms for other counties only in the transit agencies listed in question 12.
